About the Webapp

Dear Parent

Thank you for using the FORTEe web application (“Webapp”) and making a valuable contribution to the success of the FORTEe study. On this page you will find instructions on how to help your child complete the questionnaires within the FORTEe web application (https://fortee.ghh4.unimedizin-mainz.de).

If you have any questions, please contact your study centre.

Please select the questionnaire for which you would like to see the instructions:

Instructions regarding the following questionnaires for children aged 5-7-years:

  • PedsQL TM Multidimensional Fatigue Scale – YOUNG CHILD REPORT (ages 5-7)
  • PedsQL TM Paediatric Quality of Life Inventory – YOUNG CHILD REPORT (ages 5-7)
  • PedsQL TM Cancer Module – YOUNG CHILD REPORT (ages 5-7)

Show the child the template and point to the responses as you read:

  • “If it is not at all a problem for you, point to the smiling face
  • If it is sometimes a problem for you, point to the middle face
  • If it is a problem for you a lot, point to the frowning face

I will read each question. Point to the pictures to show me how much of a problem it is for you. Let’s try a practice one first.”

Ask the child to demonstrate by clicking his or her fingers to determine whether or not the question was answered correctly. Repeat the question if the child demonstrates a response that is different from his or her action.

Please be aware, that the smiley faces will not appear in the Webapp, as they are only intended as an assistance tool.

Instructions regarding the following questionnaires for children aged 8-12 years:

  • PedsQL TM Multidimensional Fatigue Scale – YOUNG CHILD REPORT (ages 8-12)
  • PedsQL TM Paediatric Quality of Life Inventory – YOUNG CHILD REPORT (ages 8-12)
  • PedsQL TM Cancer Module – YOUNG CHILD REPORT (ages 8-12)

Please read the following text to your child and show the example questions:

“You will be asked about things that might be a problem for you.

Please tell me how much of a problem each one has been for you during the past ONE month by choosing one answer per question.

There are no right or wrong answers.”

Example questions:

Please be aware, that the numbers 0 to 4 will not appear in the Webapp.

Instructions regarding the following questionnaire for 5-9-year-old children:

  • Child & Youth Resilience Measure – Revised – CYRM-R – For children aged 5 – 9 years

Please read the following to your child:

“Please choose one answer for each question.

There are no right or wrong answers.”

The questions are to be answered using the following 5-point scale. Please show this scale to your child before answering each question and ask him/her to indicate the matching smiley face.
